Integrated care for rare diseases: Bridging the gap to improve the lives of 30 million people in Europe


People living with rare diseases and their family members experience significant care coordination gaps, evidenced in this factsheet. 

Implementing integrated care, both within the health system and between health, social, and community services, is essential for enabling people living with rare diseases to achieve optimal health and enjoy their full human rights on an equal basis with others.

The outline for this factsheet was developed in the scope of the EU-funded INNOVCare project, Innovative Patient-Centred Approach for Social Care Provision to Complex Conditions. The project ran from 2015 to 2018 and focused on bridging the gaps in care coordination.
