Project Planning Toolkit for Patient Partnership
The Project Planning Toolkit is designed to support project managers to embed patient partnership into project planning and implementation processes for greater collective impact.
This Toolkit includes four distinct tools, developed in collaboration with project leads, to help project managers decide in advance how and when people living with rare diseases and their representatives will participate in the project, work collaboratively with healthcare professionals, and assess the impact of these partnerships.

Project Planning Toolkit for Patient Partnership (English)
Is this Toolkit suitable for your project?
The Toolkit is designed for projects aimed at improving healthcare services and policies, involving both patient representatives and healthcare professionals in collaboration at various stages. A pre-screening questionnaire is available to help determine if your project is a good fit for the Toolkit.
Who should use it?
This Toolkit is intended for project managers interested in developing a workplan within a project proposal to: (i) integrate patient voices at each critical phase of a project, (ii) organise patient-clinician collaboration throughout the project, and (iii) assess the impact of this collaboration in the project results. It is recommended to develop this workplan in collaboration with project leads.
What’s included?
1
Project Planner
This tool helps to identify the areas where patient representatives and health professionals will collaborate as a team and what type of involvement is needed from patient representatives throughout the project. Based on this information, it facilitates the development of a workplan to expedite this collaboration. It is designed for use by project managers, with input from project leads. This tool can be used as a standalone resource.
2
Engagement Planner
This tool helps to organise engagement with the wider patient community, beyond patient representatives. It is designed for use by patient representatives, with support from the project manager, to plan for and ensure meaningful engagement with their own community. This tool should be used once the Project Planner is completed.
3
Self-Assessment Tool
This self-assessment questionnaire guides the user in evaluating whether the project incorporates a strong patient partnership approach and allocates the necessary resources to support effective patient-clinician collaboration. It is designed for use by project managers. This tool should be used once the Project Planner is completed.
4
Impact Assessment Tool
This tool facilitates the evaluation of the impact of patient partnership in the project, once the project has finalised. It is designed for use by project managers in collaboration with the whole project team.
